Diamond Health Care Solutions Ltd is a Care Company which provides the highest home care and supported living services to people in the community to improve their independence. This is a Nurse Led Company and CQC Registered. We supply well trained staff to cater for your holistic needs at home. The model of our services is Person-centred which means that individuals will be fully involved in their care to make decisions and choices that suits their needs. Our priority is to deliver Safe, Effective, Caring and Well-Led care, protecting our clients from abuse and harm. We are unique in the market because of our innovative ways of care delivery. The company has invested in a home digital software whereby carers will log in and out of the service users' home while on duty. This app will also enable family members to monitor the care delivered to their loved ones.

Our packages start from once or twice a week to daily packages with a minimum of 30 minutes per call, 24 hours care and reablement in your home after complex hospital discharges.

We assist with personal care, shopping, mild house tasks, companionship, prompting with medications, meal preparation, pamper sessions, attending appointments, etc.

We work with service users with special needs, Alzheimer's disease, frailty due to old age, dementia, Physical disabilities, learning disabilities and old age.

Our clients are adults from 18-65 years old and 65 years and above.
